Registration

The AHSEP programs are popular and space is limited.

Enrollments are granted in the order that applications and payments are received. To enroll:

• Complete the online enrollment application*
• Print and complete the Medical Form and Waiver
• Payments may be made by check or credit card.
  • Make checks payable to AHSEP
  • For credit card payments, print and complete the credit card payment form
• Send Medical form, Waiver, and payment to:
